Achieving a specific evolution in the Digimon Survive depends on the the new evolution system implemented in the game. Basically, there are two categories for Digimons: Partners and Recruits.
In order to evolve any Digimon, you need to consider first if it's a Partner or a Recruit.
To evolve a Partner Digimon, you need affinity to a character, an ending or specific scenarios to achieve an evolution.
To evolve a Recruit Digimon, you need the Enlightment Slab. Depending on the level, you need one of the following:
For partners digimons, you evolve based on the decisions you make. This is called Karma, and it has three types: Moral, Wrathful and Harmony.
Here are all the partner rookies you can find in the game, which is our start point:
In the other hand, we have recruit digimons, who are not based on decisions but they need an Enlightenment Slab item depending on the level you are evolving into: Mature Enlightenment Slab, Perfect Enlightenment Slab and Ultimate Enlightenment Slab.
Above you will find the all requirements and the complete evolution lines and chart for each evolution in Digimon Survive.

This is Ryo Tominaga's partner. You need a specific affinity level with him to unlock new evolutions.
This is Miu Shinonome's partner. A certain affinity level with her is required to unlock new evolutions.
Recruiting a Digimon is an entirely different process. You need to actually talk to the digimon and use the right answers to specific questions in order to get them. Here are all the evolution lines for recruit digimon:
